Life Insurance:  100%-300% of salary, maximum $500,000
  Payable on death from any cause
   
Accidental Death Payable on death from any cause
& Dismemberment: Payable on accidental death
   
  Schedule of losses in the case of dismemberment
   
Dependent Life:  $10,000 upon death of spouse
  $5,000 upon death of eligible child
   
Long Term Employee Paid Premium-66.7% of the first $2500 monthly earnings, 50% balance, maximum
Disability: $6000 (non-taxable income)
  Employer Paid Premium -75% of monthly earnings, maximum $6000 (taxable income)
  Qualifying period is 120 days
  Payable while disabled, to a maximum of age 65
  Disability based on own occupation for first 24 months of disability and any occupation thereafter
   
Extended Health       Option 1 80% coverage for prescription drugs
                                       Option 2 100% coverage for prescription drugs

  dispensing fee cap of $8.00 per prescription
  lowest cost alternative drugs covered
  100% coverage for:
 
  •  semi-private hospital
  •  emergency ambulance
  • $500 maximum per calendar year per paramedical practitioner (acupuncturist, chiropodist/podiatrist, chiropractor, dietician, massage therapist, naturopath, osteopath, psychologist, speech language pathologist)
  • $600 maximum per calendar year for physiotherapist
  •  hearing aids to a maximum of $1,000 every five calendar years
  • $10,000 maximum per calendar year for home care nursing
  • outside Canada medical emergency benefits, including Global Medical Assistance
  • $300 maximum every two calendar years (every year if under age 19) for lenses, frames, contact lenses, prescription industrial safety glasses and laser eye surgery
  • $75 every two calendar years (every year if under age 19) for eye exams
Dental Care:   Option 1 80% coverage:

  • check ups and cleanings – once every six months if under age 19, once every 12 months if age 19 and older 
  • fluoride treatment provided once every six months if under age 19   
  •  extractions, fillings, endodontics and periodontics
  50% coverage for:
 
  • major dental services – dentures, crowns, bridges
  •  orthodontics - under age 19 only
  $2,500 per person per calendar year maximum for basic and major dental services combined
   
  $2,500 per dependent lifetime maximum for orthodontics
   
  Coverage provided based on usual and customary fee guide of the provincial dental association general practitioners or specialist fee guide
   
Employee and Toll free 24 hour a day access to confidential counselling service for employees and family members to assist in the resolution of personal problems or crises
