Life Insurance and Accidental Death &
Dismemberment:$70,000

Long Term Disability:
- The waiting period is 120 days or 4
months (after Employment Insurance)
- $2,000 per month for the
first 2 years, increasing to $2,500 thereafter (integrated with
other income sources to a maximum of 85% of gross or net
pre-disability income depending on tax status of benefits).
- Payable to age 65.
- CPP is not deducted from the monthly amount.
Note: To be eligible for Disability benefits, the
employee must be in "benefit" under the LTD benefits, and
actively at work at the time of disability
For Employees and Eligible Dependents-Spouse and Children
- Great West Life
Extended Health and Vision Care
- 80% coinsurance for: prescription drugs - pay-direct drug
card and dispensing fee cap of $8.00
- 100% coinsurance for: semi-private hospital; emergency Out
of Canada services ; Travel Assistance coverage, paramedical
visits, other medical services and supplies (certain maximums
and restrictions apply)
- Paramedical practitioners -$500/year per practitioner and
$600/year for physiotherapy
- $300 every 2 calendar years for glasses, contacts or laser
surgery for adults, and every year for under age 19
- $75 every 2 calendar years for eye exams for adults
Dental Care
- 80% coinsurance for: Basic Dental Services -
including check up & cleaning: age 19 and over, every calendar
year; under 19, once every 6 months, fillings, extractions, root
canals, periodontal treatment, minor denture repairs, oral
surgery
- 50% coinsurance for: Major Dental Services - dentures,
crowns and bridges
- $2,500/person /calendar year for Basic and Major Services
combined
- 50% coverage for Orthodontia (braces) for under age 19,
maximum.$2,500/dependent/lifetime

Employee and Family
Assistance Plan
Toll-free 24 hour access to confidential counselling services to assist in resolution of personal
problems or crisis. Eligibility is based on hours actually
worked. 300 hours is required to start the plan and every month
a minimum of 150 hours is deducted and the employee will be "in
benefit". Coverage can be terminated if the employee has less
than 150 hours accumulated in the Hourbank, but coverage can be
continued for up to 6 months, under a self-pay option.
If the employee has health and dental coverage from
their spouse's plan, the employee must still enrol in this plan.
The employee can coordinate claims between the 2 plans for up to
100% reimbursement, or the employee's spouse could "opt out" of
these benefits only. Life and disability benefits are only
available from the employee's own employer.
